Plans we accept
Both clinics are in-network with Medicare, Medicare Advantage, Aetna, Blue Cross Blue Shield and Cigna, plus a number of other plans. We also treat patients under workers' compensation and Florida auto injury (PIP) claims and coordinate the documentation with the adjuster or attorney. Because networks change, the front desk confirms your specific plan before you come in.
Medicaid: we are not in-network with Medicaid. Medicaid patients are welcome as self-pay, and the front desk explains the cost of a new patient visit on the call before anything is booked.
Medicare Advantage and HMO plans: many require a referral from your primary care doctor for a specialist visit, and prior authorization for injections, nerve blocks, Botox and Vivitrol. We request these for you. It can add a few days before a procedure, so we start early.

What is self-pay only
- StemWave shockwave therapy (Davie only). Insurance does not cover it. Sessions and packages are self-pay.
- PRP injections. Not covered by insurance. Self-pay, and eligible for HSA and FSA funds.
- Wellness injections and IV infusions (lipotropic injections, IV vitamin infusions). Self-pay.
- Weight loss medication when your plan does not cover it. Some commercial plans cover Wegovy or Zepbound for obesity with prior authorization. Medicare does not currently cover medication prescribed for weight loss alone. We explain the self-pay program on the call.
Self-pay is also available for any service if you are uninsured or prefer not to use insurance. Pricing is explained clearly on the phone before you book, and Medicare patients receiving a non-covered service sign an Advance Beneficiary Notice so there are no surprises.
What to bring to your first visit
- Photo ID and your insurance card (or auto claim or workers' comp claim details).
- A complete list of current medications and your pharmacy.
- Any imaging reports (MRI, X-ray, CT) and records from doctors you have already seen.
- For a referral-based plan, the referral from your primary care doctor. If you are not sure, tell us and we will check.
Do you take Medicare?
Yes, at both clinics. Original Medicare and Medicare Advantage plans. Many Advantage plans need prior authorization for procedures and we handle that.
Do you take my insurance?
We are in-network with Medicare, Medicare Advantage, Aetna, Blue Cross Blue Shield and Cigna, plus a number of other plans. Call either clinic and tell the front desk the plan name. The front desk verifies it before your appointment and tells you your expected copay or coinsurance. We are not in-network with Medicaid, but Medicaid patients are welcome as self-pay.
How much is a self-pay visit?
We do not publish prices online because they depend on the service. The front desk gives you the exact self-pay price on the phone before you book.
Do you treat workers' compensation and car accident injuries?
Yes. Bring your claim number and adjuster contact. In Florida, you must see a doctor within 14 days of a car accident to keep your PIP benefits, so call promptly.
Can I use my HSA or FSA?
Yes, for self-pay services including PRP, StemWave and wellness infusions, and for copays.
