How do I become a new patient?
Book online through Zocdoc or phone either clinic. If you call, a member of the team listens to what is going on, verifies your insurance or explains self-pay, and books your first visit.
Which clinic should I choose?
Davie, on State Road 84, serves Broward County. Boynton Beach, on West Boynton Beach Boulevard, serves Palm Beach County. The same physicians and services are available at both, except StemWave shockwave therapy, which is at Davie only.
What are your hours?
Both clinics are open Monday to Friday, 9am to 5pm.
Do I need a referral?
Not for most PPO plans or original Medicare. HMO and many Medicare Advantage plans do require a referral from your primary care doctor. Tell us your plan and we will check.
What should I bring?
Photo ID, insurance card, a list of your medications and pharmacy, and any imaging reports or records you have. For controlled medication, Florida requires an exam on the same day it is prescribed, so please plan to be examined at the first visit.
Do you speak Spanish?
Yes. The team is bilingual at both clinics.
Which insurance do you accept?
Medicare, Medicare Advantage, Aetna, Blue Cross Blue Shield and Cigna, plus a number of other plans, at both clinics. We are not in-network with Medicaid; Medicaid patients are welcome as self-pay. Insurance and self-pay details.
Pain management
Will I be prescribed opioids?
Not as a first step. We start with non-controlled medication and procedures such as injections and nerve blocks. When a controlled medication is appropriate, it is prescribed within a written agreement with regular follow-up and monitoring, as Florida law requires. Read about medication management.
Do injections hurt?
Trigger point and joint injections take a few minutes and most patients describe a brief sting and some pressure. Soreness for a day or two afterwards is common and fades.
Do you do epidural injections or spinal cord stimulators?
Not in the office. When imaging and symptoms call for an epidural steroid injection or a stimulator trial, we coordinate the referral and continue managing your care.
What is StemWave and is it covered?
StemWave is an acoustic shockwave therapy for tendon, muscle and joint pain, available at Davie. It is not covered by insurance. Read more.
Neurology and migraine
Does insurance cover Botox for migraines?
Yes, with prior authorization, when you have 15 or more headache days a month and have tried preventive medication. Approval usually takes 7 to 10 business days and the injection is scheduled after it. Read more.
Do you do EMG or nerve conduction studies?
Yes, as part of a neurology work-up for numbness, tingling and neuropathy when the doctor considers it necessary.
Detox and addiction medicine
Is this a rehab?
No. AffordaHealth is a private physician practice providing office-based, outpatient treatment. You live at home, keep working and come in for scheduled visits. People who are medically unstable or at risk of severe withdrawal are referred to hospital-level care first.
Is my treatment confidential?
Yes. Substance use treatment records have additional federal protections beyond standard medical privacy, and nothing is shared without your written consent except where the law requires it.
Weight loss
Does insurance cover semaglutide or tirzepatide for weight loss?
Some commercial plans cover Wegovy or Zepbound for obesity with prior authorization. Medicare does not currently cover medication prescribed for weight loss alone. Self-pay programs are available and explained on the call.
Do I have to come in every week?
No. The injection is self-administered at home once a week. Follow-up visits are typically monthly for dose adjustment and monitoring.
